Abstract (AI)
PURPOSE: The purpose of this study was to assess the effect of overnight bladder drainage (OBD) on upper and lower urinary tracts of boys after endoscopic valve ablation (EVA). MATERIALS AND METHODS: Tc-dimercaptosuccinic acid scan, febrile urinary tract infections (UTIs), and tolerability to OBD. RESULTS: A total of 99 patients, 47 patients with OBD and 52 patients with no OBD, were included between September 2021 and August 2023. OBD showed significant improvement in HN, VUR, renal parenchymal area, bladder capacity, outline, detrusor overactivity, compliance, daytime continence, and nocturnal polyuria. However, no significant difference was noted regarding bladder wall thickness, postvoid residual urine, bladder sensation, detrusor leak point pressure, maximum flow rate, flow curve morphology, renal function, renal scarring, and febrile UTIs. Tolerability to OBD was reported in 87%. CONCLUSIONS: OBD, apart from patients' tolerability, improves HN, VUR, renal parenchymal growth, abnormal bladder morphology and dynamics, daytime continence, and nocturnal polyuria that persist after EVA with no consequent renal function affection or febrile UTIs.
Key Findings
1
In a randomized controlled trial of 99 boys after endoscopic valve ablation, overnight bladder drainage significantly improved hydronephrosis and vesicoureteral reflux.
2
No significant differences were observed in renal function, renal scarring, febrile urinary tract infections, bladder wall thickness, residual urine, or several urodynamic parameters.
3
Overnight bladder drainage significantly improved renal parenchymal area, bladder capacity and morphology, detrusor overactivity, compliance, daytime continence, and nocturnal polyuria.
4
Overnight bladder drainage was tolerated by 87% of patients.
5
The intervention improved persistent upper- and lower-tract abnormalities after ablation without adversely affecting renal function or increasing febrile urinary tract infections.
Research Object
Boys with posterior urethral valves after endoscopic valve ablation, including their upper and lower urinary tracts
Research Subject
Efficacy and tolerability of overnight bladder drainage in improving post-ablation hydronephrosis, vesicoureteral reflux, renal parenchymal growth, bladder morphology and dynamics, daytime continence, and nocturnal polyuria without impairing renal function or increasing febrile UTIs
